Enrollment Instructions for Accepted Applicants,
Fall 2011
Undergraduate Orientation Schedule Fall 2011
Graduate Orientation Schedule Fall 2011

Greetings! Below is a step-by-step list of instructions to confirm your enrollment and get prepared for your arrival to San Francisco. We look forward to working with you and welcoming you to the Conservatory community.

FORMS TO COMPLETE AND FINAL APPLICATION REQUIREMENTS

1. Enrollment Card: Please complete this card and mail it back to us. Graduate applicants should return this by April 15 and undergraduate applicants by May 1. If you receive your offer of admission or financial aid/scholarship award after May 1, please return this enrollment card within two weeks of receiving of those documents.

2. Deposit Card: We require a deposit of $500 along with your enrollment card to confirm your acceptance of admission. You may pay by credit card or check (made payable to SFCM). Please include this card and payment with your enrollment card.

3. New Student Billing Address Form: Please fill out this form to the best of your ability. Students will often provide a permanent address to use until they have found housing in the Bay Area. You will have an opportunity to update your billing address during registration. International applicants can wait to submit this until they have found housing in San Francisco.

4. Medical Insurance Waiver Form: Health insurance is required for all full-time students and offered through the school. If you already have coverage through an insurance provider, please complete this form in order to waive charges for insurance premiums.

5. Final Transcripts: If you are completing either a bachelor’s or master’s degree this spring, you are required to request an official transcript that verifies your date of graduation. This is mandatory in order for you to enroll in classes this fall. For those of you who already completed your degrees, please email us at admit@sfcm.edu to confirm that we have a transcript on file with your date of graduation stated.

6. TOEFL Requirements for International Students: Applicants who were admitted on "TOEFL Probation" must submit either a passing score of 61 or higher or documentation that you completed a Level 109 course at an ELS center. We need one of these documents in order to process your I-20.

All forms and transcripts can be mailed to: San Francisco Conservatory of Music
50 Oak Street
San Francisco CA 94102
